Carevium is a cloud based digital health management platform for elder care industry. Carevium is used by assisted living facilities, memory care / Alzheimer’s care communities, adult day care centers, independent senior living communities, and home care service agencies.
The platform can also be used for chronic disease management and patient engagement by ACO participants.
Carevium is used as a care coordination and care management platform by professional caregivers. Carevium interfaces with pharmacies and health monitoring devices such as pulse oximeters, glucometers, blood pressure monitors, and smart scales to measure and record vital health monitoring parameters. Hospitals and home care agencies can leverage Carevium platform for remote home care management and care coordination. The system includes video chat and private messaging to connect seniors, caregivers, care coordinators and family members.
